Policies *

Booking

Please fill out a service form or wedding service form to inquire. Allow at least 48hrs for an email response. Large projects that require custom materials such as prosthetics, contacts, teeth, and/or other accessories must be booked at least two-three weeks in advance to allow for ample shipping times.

Forms & Waivers

Each client is required to fill out a service form in order to book. This is to ensure all necessary client information is gathered, and that all service details are gathered, and policies are stated clearly and acknowledged. These forms must be completed upon booking along with the booking retainer payment to reserve your service.

24 hours before the booking time each client being serviced will be emailed a consent form/ waiver. This must be completed before being serviced. (Does not apply to event/ party services)

Minors must have a guardian present to sign a consent form/ waiver in person before being serviced. Please see “Underage clients” for more information. (Does not apply to event/ party services)

Payment

Payments can be made via Venmo, Cashapp, PayPal, or cash (for remaining payment only) Full payment must be made at the end of the service. No exceptions. Please reach out to Katie for payment information.

Booking Retainer

A non-refundable 50% booking retainer is required to reserve your booking.

For services charged by the hour, a one hour minimum will be charged as the booking retainer.

For Weddings a 25% booking retainer will be due. If 25% of the total service cost is less than $150 then $150 will be due as a the booking retainer

Dates and times will not be held without booking retainer payment. Payment can be made via Venmo, Cashapp, or PayPal.

Cancellations

Cancellations are to be made at least 48 hours in advance. Booking retainers are non-refundable. Any other payment(s) made after the initial booking retainer are non-refundable if the cancellation is made less than 48 hour advance notice.

Wedding Cancellations must be made with at least 60 day notice. Booking retainers are non-refundable. Any other payment(s) made after the initial booking retainer are non-refundable if the cancellation is made with less than 60 day notice

Cancellations with more than 48 hour/ 60 day notice will be refunded any payment(s) made after the initial booking retainer.

Refunds

Booking retainers are non-refundable. If a service results in irritation, discomfort, allergic reaction, etc. you will be advised to seek professional medical attention before being serviced again. For these reasons, each client is required to fill out a consent/ waiver form 24 hours prior to their appointment. Any payment(s) made after the initial booking retainer but prior to the appointment will be non refundable in the case of a no show or cancellation less than 48 hours notice.

Underage Clients

Clients under the age of 18 will require guardian consent before being serviced. Guardian consent forms are available in person and must be filled out at the start time of the appointment. The client is still required to fill out the client consent form. (This does not apply to event/ party services)

For certain services, clients under the age of 16 may not be serviced.

Please reach out to Katie in advance to confirm that the service being booked is suitable for those under the age of 16.

Behavior

Behavior including racism, sexism, homophobic language or actions, transphobic language or actions, harassment, inappropriate language or actions, hate speech, sexual harassment, or any other behavior deemed unacceptable will not be tolerated. These actions will lead to cancellation of service and/or a ban from booking future services. I strive to create a safe and comfortable environment for all my clients and will do everything in my power to maintain it as such.
